Death's Head Tree
Traits
False Appearance. While motionless, it is indistinguishable from a dead, bare tree (DC 15 Intelligence (Nature)).
Aura of Dread. Every enemy that starts its turn within 20 feet of the tree must make a DC 14 Wisdom save. Failure: It is Frightened until the end of its next turn. A creature that succeeds on the save is not immune for 24 hours — it must save again every turn.
Rooted. It can't be knocked Prone or forcibly moved.
Actions
Multiattack. It makes two Branch attacks and uses Drop Skull.
Branch. Melee Attack Roll: +8, reach 15 ft. Hit: 16 (2d10 + 5) Bludgeoning damage.
Drop Skull. The tree drops one of the skulls hanging from its branches. Every creature within a 10-foot-radius Sphere must make a DC 15 Dexterity save.
- Failure: 17 (5d6) Necrotic damage, and the target's Hit Point maximum is reduced by the same amount.
- Success: Half damage, no reduction to HP maximum.
A reduced HP maximum is restored when the target finishes a Long Rest, or via Greater Restoration or a similar spell.
Wail of the Fallen (1/Day). All the skulls on the tree scream at once. Every enemy within 60 feet that can hear the tree must make a DC 15 Wisdom save.
- Failure: Frightened for 1 minute, and also Incapacitated during its first turn. The target can repeat the save at the end of each of its turns.
- Success: Frightened (not Incapacitated), and it can repeat the save at the end of its turn.
How to Use
This is a Ravenloft/horror-themed creature. Combat mechanics are secondary — the real point is atmosphere.
Scene setup:
"A bare tree stands in the mist. Hanging from its branches... fruit. As you approach, you realize: skulls. Human skulls. And one is looking at you."
Aura of Dread re-triggers every turn — there's no immunity. This keeps the party constantly Frightened: a Frightened character can't approach the tree and their attacks have Disadvantage. This breaks the "close in and hit it" tactic.
Solutions:
- Fire (Vulnerability) — double damage. A single Fireball guts it.
- Attacking from range — Frightened prevents approaching, but not attacking from range.
- Immunity to Frightened — a Paladin's Aura of Courage (Lv10) completely neutralizes this creature. This is a good thing — the Paladin player gets their heroic moment.
Balance Note
Reducing HP maximum is a heavy mechanic. I limited it: it's restored with a Long Rest (not the classic "permanent" version). Still, Drop Skull is used every turn, so in a long fight the party wears down seriously. This is deliberate — the Death's Head Tree is a siege battle creature and should end quickly.
Wail of the Fallen is once per day and includes Incapacitated — this is very strong. Use it at the start of the fight (dramatic) or when HP drops to half (a moment of despair). Both work well.
