False Appearance. Indistinguishable from an ordinary beggar (DC 15 INT (Medicine or Religion)). Contagion Aura. An enemy that ends its turn within 10 feet makes a DC 13 CON save. Failure: Gains 1 level of Exhaustion (this Exhaustion is reduced by 1 with a Long Rest).

Multiattack. One Rotting Touch and one Plaguesong. Rotting Touch. Melee Attack Roll: +3, reach 5 ft. Hit: 12 (3d6 + 2) Necrotic damage, and the target's HP maximum is reduced by the same amount (restored with a Long Rest). Plaguesong. Constitution Saving Throw: DC 13, two creatures that can hear the Plaguesong Beggar within 30 feet. Failure: The target has the Poisoned condition (1 min) and takes 5 (2d4) Necrotic damage at the start of its turn. The target can repeat the save at the end of each of its turns. Wave of Filth (Recharge 5–6). A 20-foot cone, DC 13 CON save. Failure: 21 (6d6) Poison damage, Poisoned (1 min), and Prone. Success: Half.
Key
Keep your distance (Contagion Aura) and become Deafened (Plaguesong requires "can hear"). This is the urban campaign's low-level first threat — the party learns what an "ordinary beggar" can be.
